Team Registration Forms and Waiver forms have been uploaded.
Team spots will not be reserved unless a signed waiver form is included.
If emailing the waiver form, please print a copy, have each team member print and sign his or her name, and scan the form to attach to the email.
Online versions of the tournament rules and regulations are also available to review.
